Tenant Utility Service Contract This is the service contract for a TENANT. Please complete and bring with you to the office of Utility Billing, 100 S. Market Street, Troy, Ohio (first floor). Having this form completed will expedite the sign-up process. In addition to the Contract, you must provide the following: * Copy of lease agreement * Any adult sharing the living space must appear and present acceptable proof of identification. Acceptable proof includes Driver’s License or State ID card. The form is in PDF format. PDF forms require Acrobat Reader to be installed on your computer.
