Owner Utility Service Contract This is the service contract for a PROPERTY OWNER. Please complete and bring with you to the office of Utility Billing, 100 S. Market Street, Troy, Ohio (first floor). Having this form completed will expedite the sign-up process. In addition to the Contract, you must provide one of the following: * Copy of deed showing property ownership in the name of the applicant(s); * Copy of closing papers from title company showing property ownership in the name of the applicant(s) * If the property has not yet closed, a copy of the accepted Contract to Purchase showing the name(s) of the intended purchaser(s) of the subject property. Be sure to bring acceptable proof of identification. Acceptable proof includes a Driver’s License or State ID card. The form is in PDF format. PDF forms require Acrobat Reader to be installed on your computer. You probably already have it, but you can get a free copy or upgrade here. This is a fillable form, and can be completed as viewed on your computer.
